The purpose of these questions is to document and compare your symptoms at different times in your life. This comparison is based on how you recall feeling at particular times/dates. You will be asked to provide approximate dates.

Two identical sets of 10 questions will be shown to you twice. The first set refers to your symptoms when you felt better whether that is now or was in the past. When you have completed the first set of questions, the next 10 questions will be shown to you. For these questions, recall what your symptoms were like when they were worse, whether now or in the past.

Next, the program will automatically draw your responses and show them to you. The final result will be two diagrams as illustrated here—a red diagram representing your symptoms when worse and a green diagram representing your symptoms when they are better.
